How is Ksharasutra done?

In Fistula-in-ano, a malleable probe with the Ksharasutra in its groove is passed through the external opening of the fistula to the internal one. The Ksharasutra is then tied around the fistulous tract and left in place.

In Fissure-in-ano, the Ksharasutra is applied around the anal sphincter after making a small incision at the posterior midline.

  The Ksharasutra is replaced with a new one every week until complete healing. The patient can resume normal activities partially within 5-6 hours, and usual routine within 4-5 days after the procedure.


How does Ksharasutra work?

Ksharasutra works by applying pressure through the ligation of the thread. This causes mechanical strangulation of blood vessels and tissues, leading to pressure necrosis of any swelling, such as piles or fistula. The thread acts like a vehicle for the medicines like snuhi, apamarga, haridhra etc., to reach the site by coating on the thread rendering anti-inflammatory, anti-microbial, wound healing activities in the disease tracts.
